Onychomycosis typically presents as a progressive infection of the nail unit. Pharmacists should be adept at recognizing the clinical subtypes, most commonly Distal Lateral Subungual Onychomycosis (DLSO).<\/span><\/p>

  • Leukonychia<\/strong>: Often presents initially as white or yellow opacities in the nail plate (specifically in White Superficial Onychomycosis or early DLSO).<\/li><\/ul>

    Progressive Pathology:\n<\/h2>\t\t\t\t<\/div>\n\t\t\t\t

    • Chromonychia:<\/strong> Discoloration ranging from yellow\/white to brown.<\/span><\/li>
    • Subungual Hyperkeratosis:<\/strong> Accumulation of debris under the nail plate leading to thickening (onychauxis).\u00a0<\/span><\/li>
    • Onycholysis:<\/strong> Detachment of the nail plate from the nail bed.<\/span><\/li>
    • Dystrophy:<\/strong> Crumbling, splitting, or total destruction of the nail plate.<\/span><\/li>
    • Odor:<\/strong> Malodorous debris may be present due to secondary bacterial colonization or fungal metabolic byproducts.<\/span><\/li><\/ul>

      Onychomycosis is a fungal infection of the nail apparatus (matrix, bed, or plate). Pathogens generally invade the hyponychium or lateral nail folds.<\/p>

        • Trichophyton rubrum<\/em><\/strong> is the primary pathogen.1<\/sup><\/span><\/li>\n \t
        • Trichophyton mentagrophytes<\/strong><\/em> is the second most common<\/span><\/li>\n<\/ul>\n<\/li>\n \t
        • Non-Dermatophyte Molds (NDMs) & Yeasts:<\/span><\/strong>\n

          • Candida<\/strong> <\/em>species (more common in fingernail infections or chronic<\/span><\/li>\n \t
          • NDMs such as <\/span>Scopulariopsis<\/span><\/strong><\/em>, Fusarium,<\/strong><\/em> and Aspergillus<\/strong> <\/em>species are causative in a minority of cases but may be resistant to standard <\/span>therapies 1<\/sup>.<\/span>\n\n<\/span>\n\n<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t

            • Trauma:<\/strong> Prior history of micro- or macro-trauma to the nail unit creates portals of entry.<\/span><\/li>\n \t
            • Immunosuppression:<\/strong> HIV, chemotherapy, or chronic corticosteroid use.<\/span><\/li>\n \t
            • Diabetes Mellitus:<\/strong> Patients with diabetes are at significantly higher risk due to neuropathic and vascular changes. Studies indicate that approximately one-third of diabetic patients suffer from onychomycosis.2<\/sup><\/span><\/li>\n \t
